now i'm pretty busy. monday to friday morning, i went to adi husada for my "magang", and then on thursday to saturday, i went to college. it was a different world. at first, i was itchy to help at the ER. but i was there to study the hospital administration, not the medical service. it was hard just sitting there and do nothing. but sometimes i gave up and helped with the paper work. i didn't interact with the patient directly, but i had something to do beside waiting. the college was something else. at first, i didn't know a thing about the lecture. i had no idea. it was another world for me. all those foreign terms and the bureaucracy made me dizzy. but i got the hang of it.
my favorite course is logistics. the lecturer was very clear in teaching us, it made it interesting and easy to understand. my least favorite course was nursing management and human resource. the first one, the lecture was sooo boring. i didn't understand a bit. the second one, the first lecture was confusing, the second week was okay, but the third week turned out confusing again, and the last one was very good. so, it was a love-hate situation for me. but a little by little, i started to enjoy the confusion. hahaha..
and for the "magang", if the unit manager was okay, it was fun to spend the time. but if the manager wasn't okay, it was excruciating spending the time there. and all the reports were miserable. but i still hope for the best. until now, it was okay at the new hospital. people were nice and helpful. i hoped it will stay the same.

so long, mitra
many people asked me why i haven't quit my job at RSMKS yet. as far as everybody's knowledge, working at mitra mean a hell lot of job but small wages. well.. let's review it from my point of view.
after finishing my medical school, the only thing in my mind was finding a job as quick as i could. since i'm firstborn but almost last to graduate (due to longer studies - 6 years to finish med school), i had to catch up my sisters pretty quick. i was already giving piano lesson at that time, but it was just a side job. so when my friend told me that there was opening at RSMKW, i applied there with couple of my friends. we were called to be tested and interviewed. everybody got home after finishing their test, but i had to wait to be interviewed on the same day. the director noticed that my house was on west surabaya, so she asked whether or not i'm willing to be transferred to RSMKS since they were also in short of person. since my first intention was to get a job soon, i agreed to it. so she arranged for me to be interviewed again by the other director.
at that time, i questioned my decision. some of my friend has been accepted at RSMKW and already working, while i'm still waiting to be called again. i was in doubt, is it true that they still need someone? or have they gotten the person they need. but finally i got the call. i met with the director and the head of ER, and i got accepted! hahaa.. though at that time i was really a fresh graduate, no experience at all, and haven't been trained anything yet (ACLS, ATLS, etc). fiuh.. that time, i was relieve. God heard my prayer.
day by day, i started to enjoy it. though it was hard work (really2.. i don't understand that those so called high-end patients were very demanding and sometimes stupid.. ) but i like all the people there. they were warm and nice. i loved my colleagues.. all the nurse.. in short, i could blend with them easily. since i was considered to be quick learner, they gave me more schedule in ER as second doctor more than my friend (at that time, there were two of us who got accepted at the same time). and i got flying solo almost the same as my friend who were there a month earlier.
i remembered, i was pretty scared when i was flying solo. i prayed to God that everyone will be just fine and healthy. no stroke attack, no heart attack, no accident. hahaha.. indeed, my first flying solo was fine. it was quiet. and it also happened to my second, third, and so on. but as time passed, i faced some emergencies, i made some mistakes, and i almost got sued! but i passed it all. i even cried when i found out that i couldn't save the old man on my night duty, though i worked my ass of to save his life. well, i was exhausted at that time and got attached easily to him (which i shouldn't do). i got my ups and downs here in mitra.
but back to my very first reason. i applied for the job for one main reason: to learn. being a doctor is a never ending job. you never know what you'll be facing, you couldn't predict what will come through the ER door everyday. though it might be same cases, there would be hundreds outcome. it depends on your decision. you have to be quick and intuitive. and sometimes, just be stubborn. since my first reason is to learn, money came last.
i admit, i was disappointed when i found out about my first month salary. but then, when i got more responsibilities, though it mean more hard works, it also mean more money. i remembered i got almost twice my usual salary when i really worked my ass off. no day off. but i could pass all that. and the benefit? more people got to know me. more people got to trust me. and i also gained more confidence.
so, for those who kept asking why i still at mitra, i guessed i have answered your question. if money was my orientation, i would've quit my job from the very first time i got my first salary. so now, i quit because i wanted to learn more. and i couldn't get it at mitra, so i guess i have to say goodbye for now. i really thank everyone there, who had taught me and taken care of me, and apologized if i had cause them troubles. wish you all good luck. and wish me good luck too..
and perhaps, if God permit, i would be back..

people..oh..people...
oke, sapa sih yg ga suka barang gratisan? tp kalo minta gratis melulu ini kenemenen. i'm talking abt patients. our patients back in the hospital. i guess the insurance company has grown a lot since there are more patiens with health insurance coming to our hospital. of course it seems like a good thing, people have steady income so they can save more and invest in that kind of insurance. but it's also had bad side effect. one of the administration officer told me, that probably they have to pay for the patient's health cost since he was out of limit and they couldn't get hold of him. how shame!! kalo orangnya baca, NGACA DEH LU.. gayanya aja sok kaya, tp kalo udah ditagih begini sok sibuk, ga kasihan apa ama orang counter? berapa lo gaji mereka? apa salah mereka? sampe harus nomboki biaya orang lain yg jelas sebetulnya mampu utk bayar sendiri tp krn merasa udah bayar premi, pasti semuanya bisa gratis, tanpa menyadari kalo limit mereka itu bisa habis.
type 1 : ga nyadar kalo limit habis
mrk dateng dg sok sakit berat, minta segera dilayani. berhubung kita ini melayani jasa, pasien dipersilakan masuk utk diperiksa. some insurance came with cards, yg harus digesek. masalahnya adalah, hari itu mesinnya trouble, kartunya ga bisa digesek jd petugas administrasi tdk tahu apakah limit orang tersebut cukup. sooo... after he's being examined and given prescription by the doctor, the administrator told him that his card was in trouble and cannot be used. he refused to pay first, dg alasan TOH DIA SUDAH PNY ASURANSI ITU, hubungi aja petugas asuransinya. and then he left. ternyata setelah mesinnya beres, ketahuan kalau limit orang itu habis. petugas asuransi yg dihubungi jelas tdk membantu, hny menyarankan utk menagih yg bersangkutan langsung. sdgkan yg bersangkutan, dihubungi pihak rumah sakit selalu menyatakan tidak ada di tempat, sedang keluar kota, bny alasan dan tdk mau datang ke rmh sakit. should we send you debt collector? biar Tuhan sendiri yg membalas.
type 2 : sakit ngerombong
ini tipikal paling sering. dan biasanya terjadi di awal atau akhir tahun di mana limit baru masih banyak atau masih bny sisa limit sehingga rasanya "eman2" kalo ga dipake. sound ironic, huh? yg ga sakit mengaku sakit spy ga rugi.. biasanya mereka hny sakit ringan lalu minta opname agar ditanggung asuransi, atau mengeluh sakit sekujur badan, bahkan kalau perlu satu keluarga sambatannya disampaikan semua spy obatnya banyak. bahkan ada yg terang2 bilang minta obat buat persediaan. (-.-") yg tipikal begini ini yg repot. tipikal orang ga mau rugi. yg paling baru itu kemaren, minta obat simptomatis dlm jumlah ga wajar. setelah aku tolak dan aku sinisi, eh.. malah bilang kalo dirinya sesama dokter. ehhh.. apa ndak malu? ngapain ga pake resep sendiri aja? jawabnya: krn ga gratis. ga tau apa kalo kita ini jg pny peraturan? kalo aku yg kena peringatan dr rumah sakit, emang situ mau ikut tanggung jawab? bodo amat sesama TS.. kalo ga tau aturan kyk gitu yaaaa males banget dehh...
type 3 : pokoke kudu gratis
ini cm terjadi utk beberapa asuransi yg aturannya ruwet, salah satunya Inh***th. mereka pny peraturan bahwa px yg bisa dilayani tanpa membawa pengantar adalah pasien yg emergency, dan kriteria emergency itu ditentukan oleh dokter yg memeriksa. this just happened yesterday. a woman came with her son. she complained shortness of breath. we gave her oxygen, and after i examined her, no rales nor wheezing, hence, probably she was just suffering mucus plug due to her upper respiratory infection. turned out the day before, she only got cough suppresant without expectorant or mucolytic. probably that caused she couldn't cough her sputum. her son already been given instruction for nebulizer, but they left it at home. so, it was clear that it wasn't a emergency case. the father has been told that both case are not emergency cases. he cancelled her son's bills and then left. jelas dia ga mau bayar. sedangkan sang ibu, sudah terlanjur mendapat nebulasi di UGD. at the end, she complained at us since she had to pay. dg entengnya dia protes "kenapa kok ga transparan aja di depan kalau ini bukan emergency? kan saya jadi hrs bayar.. kalau saya hrs bawa pengantar, saya bisa besok kembali dg pengantar" oke.. we are doctors and medical staff, we have no clue about administration. kalau ada pasien, ya kita layani. urusan mereka hrs bayar atau nggak itu kan urusan administrasi. apa etis kalau ada pasien yg mengeluh sesak, lalu setelah diperiksa kita tawari "bu, ini bukan emergensi, ibu mau tetep berobat atau pulang aja? nanti hrs bayar sendiri lo.." please deh.. kalo ga dilayani complain katanya we don't care. dilayani jg complain, krn mereka jd harus bayar sendiri. and FYI, si ibu setelah komplain begitu, langsung ngeloyor pulang tanpa membayar jasa. biar Tuhan sendiri yang membalas...

facts around us
ok. entah kenapa akhir2 ini aku lbh mudah jengkel mendengar jawaban para pasien2ku. kadang jawaban mereka itu lucu, kadang sok tahu tapi bodoh. awalnya takkira hanya golongan menengah ke bawah saja yg akan menjawab seperti itu, mengingat mungkin mereka tidak rutin mengupdate isi otak mereka dg berita di TV atau di koran. atau mereka terlalu sibuk mendengar gosip artis daripada mengikuti perkembangan dunia kesehatan. tapi ternyata, di rumah sakit pun aku mendengar jawaban serupa. pingin rasanya taktabok orang2 itu. aku jd bingung. zaman semakin canggih, internet di mana2, tp mereka masih percaya hal2 yg tdk masuk akal.

#1 me: sudah minum obat apa pak/bu?
px: ya..minum obat2 biasa itu lo.. (see? obat apa sih yg biasa dan luar biasa? kalo beli sendiri itu obat biasa? kalo dr dokter itu luar biasa? apa coba maksudnyaaa???)
#2 me: selain obat batuk pilek, sudah minum apa lagi?
px: minum super tetra/amox
me: kenapa?
px: lha soalnya bukannya kalo blablablaa obatnya itu ya?
(FYI, yg minum super tetra itu cuma batuk pilek, dan yg minum amox itu mengakui kalo sebetulnya sdh ga mempan minum amox, tp krn yg di rumah cm ada itu, ya diminum dulu sebagai pertolongan pertama. OMG.. sejak kapan antibiotik itu pertolongan pertama? ga baca di koran ya? amoxicilin udah dinyatain resisten utk sebagian besar wilayah indo, apalagi buat surabaya yg tergolong kota besar.. so people.. kasihanilah tubuhmu sendiri, don't drink antibiotic unless your doctor say so.. jgn memperburuk resistensi antibiotik negara kita yg udah parah ini...)
#3 me: udah berapa biji minum amoxnya?
px: baru 2..
me: baru dr kemaren berarti? masih ada sisanya?
px: nda.. ya kalo sakit baru saya minum, dan cm tinggal 2 di rumah
(how comeee??? antibiotic is not a symptomatic drugs, so you have to take it as instructed. and how come you have spare antibiotic at home? you should take them all.. for at least 3-5 days depend on how bad your infections are)
#4 me: (px baru saja menyebut nama obat yg ga biasa diminum orang awam) dikasih siapa pak?
px: saya diksh tau orang apotek, katanya kalo sakit begini ya obatnya ini.. tp kok ga manjur ya?
me: minumnya gimana?

me: makanya pak, jgn percaya omongan orang kalo bkn dr dokter, aturan minumnya salah, ya ga manjur

px: saya dibilangi temen saya, tempo hari dia sakit kyk gini sembuh pake obat ini..

px: ya kan dosisnya keras ya dok.. ya saya cm minum 1x/hr
me: pak, dosisnya itu sudah sesuai utk orang dewasa, dan harusnya minum sehari 2 kali.

me (separo mo menepuk jidat): emang ga ada etiket obatnya bu?
px: saya ga kelihatan dok.. abis gitu etiketnya hilang, kan cuma dijepret gitu..
me: bu, obat itu harusnya minumnya cuma sehari sekali... ya pantes aja perutnya ga kuat..
#6 px: saya ini kapok dok, tempo hr minum obat sampe ndredeg jantung saya...
me: trus?
px: makanya sekarang saya minum amoxnya sehari cuma berani sekali aja, itu aja ga tiap hari
me: kenapa?
px: ya itu tadi.. takut alergi..
me: dulu2 minum amox gpp? tau kalo amox hrsnya minum sehari 3x?
px: dulu2 ga papa.. tp gara2 ndredeg itu tadi lo dok, saya takut kalo minum amox jg sama.. tau sih, biasanya sehari tiga kali..
me (mbatin dalem ati): terusin ajaaa... ga baca koran apa??? pengen tuh kyk yg steven johnson di koran2? ato yg IgE something2 yg tempo hr msk koran itu? itu kan gara2 minum obat ga sesuai aturan terus2an... mo jd korban berikutnya? ntar ujung2nya nuntut dokternya.. bilangnya malpraktek.. sekarang yg salah dokternya apa pasiennya sih?
#7 me: pak, tekanan darahnya biasanya berapa?
px: kenapa?
me: ini kok tinggi.. 160/100.. dulu2 berapa biasanya?
px: ya emang segitu.. kadang 150.. kadang bisa sampe 180
me: ga dikasih obat darah tinggi pak?
px: dikasih si dok, tapi saya ga mau biasain minum obat terus2an..
*sigh* ini lagi paradigma keliru di masyarakat. lek sakit kepala, minum mixagrip ben hari dianggap biasa. tp kalo waktunya kena darah tinggi, asam urat, kolesterol, disuruh minum obat tiap hari malah dianggep racun.. ga baik untuk ginjal. FYI people, you have to take those medicine regularly..or else, your kidney will fail due to your illness, not your medication. ntar lek udah waktunya cuci darah baru ngedumel, mengeluh dokternya ga becus, mengeluh dokternya ga informatif..
#8 tapi gimana2 paling jengkel kalo dapet pasien tipe yg 2 biji ini...
me: sudah minum obat?
px: sudah..
me: obat apa aja? dibawa nggak?
px: waduh, saya lupa namanya.. pokoknya satu buat sakit kepala ato meriang, yg satunya bapil, trus ada antibiotik juga.. wah.. nda saya bawa dok.. lupaaa.. tp gpp dok, ganti aja.. wong obatnya ga manjur kok
me: lha iya bu, saya juga maunya ganti, tp diganti apa kalo asalnya saja saya tdk tahu ibu seblmnya dapat obat apa.. kalo obatnya kebetulan sama isinya kan ya sama aja.. apalagi ibu ga berobat ke sini sebelumnya, tidak ada datanya di sini.. saya taunya dr mana coba?
(batinku, emang gue peramal? bisa tau situ sebelumnya minum apaan?)

me: ya kalo saya ga tau nama obatnya atau isinya gimana? ibu kok ga dicatat nama obatnya? begini ini penting lo bu.. kan bahaya, kalo cm gatal ama bentol2 aja sih gak papa, tp kalo sampai shock anafilaktik itu bisa bahaya. hitungan detik ibu bisa meninggal..
px: yaa... pokoknya saya ada alergi dok..
*capek deeeee* emg tangan gue scanner? scan di jidat akan keluar nama obat yg alergi? atau punya bola kristal? tinggal tanya bola kristal, pasienku alergi obat apa ya? trus kluar jawabannya? gitu?? so people, please do remember what you are allergic to.. this is very important. if you can't remember the ingredients, just remember the name. or if you can't remember it, write it down in a piece of paper, put it in your wallet or in your note on your mobile phone or your tablet. or even better, keep the wrapping! whatever it is.. please do remember your allergy. blame us if we have already known it and still gave it to you. don't blame us if you don't even know what your allergy is and got allergy reaction after taking the medicine.

px: obat ini..ini..ini... kemaren kan saya udah ngantuk dok, jadinya baru tadi pagi sekali saya minum. tapi ga ada efeknya tuh.. udah ganti aja.. wong kemaren saya periksanya cuma ke dokter praktekan dekat rumah aja..

apalagi kalo ada yg namanya infeksi. udah dibiarin berapa hari tuh infeksi? eh..malah minta sekali minum obat lgsg beres..
oke.. sementara hanya itu yg bisa takinget.. ntar kalo ada reaksi lucu2 lagi (menurut sudut pandangku) akan aku tulis lagi. untuk sementara, itu dulu. ambil poin pentingnya, jadikan peringatan. tularkan kebiasaan peduli kesehatan ke orang lain. sehat itu mahal. dan jgn protes menganggap kita mata duitan krn bilang sehat itu mahal. sering jadi mahal krn pasien sendiri yg tdk aware and tdk mau menjaga kesehatannya sejak terdeteksi dini, akhirnya malah terjadi komplikasi dan biayanya otomatis membengkak.

px: ya..minum obat2 biasa itu lo.. (see? obat apa sih yg biasa dan luar biasa? kalo beli sendiri itu obat biasa? kalo dr dokter itu luar biasa? apa coba maksudnyaaa???)
#2 me: selain obat batuk pilek, sudah minum apa lagi?
px: minum super tetra/amox

px: lha soalnya bukannya kalo blablablaa obatnya itu ya?
(FYI, yg minum super tetra itu cuma batuk pilek, dan yg minum amox itu mengakui kalo sebetulnya sdh ga mempan minum amox, tp krn yg di rumah cm ada itu, ya diminum dulu sebagai pertolongan pertama. OMG.. sejak kapan antibiotik itu pertolongan pertama? ga baca di koran ya? amoxicilin udah dinyatain resisten utk sebagian besar wilayah indo, apalagi buat surabaya yg tergolong kota besar.. so people.. kasihanilah tubuhmu sendiri, don't drink antibiotic unless your doctor say so.. jgn memperburuk resistensi antibiotik negara kita yg udah parah ini...)

(how comeee??? antibiotic is not a symptomatic drugs, so you have to take it as instructed. and how come you have spare antibiotic at home? you should take them all.. for at least 3-5 days depend on how bad your infections are)

*sigh* ini lagi paradigma keliru di masyarakat. lek sakit kepala, minum mixagrip ben hari dianggap biasa. tp kalo waktunya kena darah tinggi, asam urat, kolesterol, disuruh minum obat tiap hari malah dianggep racun.. ga baik untuk ginjal. FYI people, you have to take those medicine regularly..or else, your kidney will fail due to your illness, not your medication. ntar lek udah waktunya cuci darah baru ngedumel, mengeluh dokternya ga becus, mengeluh dokternya ga informatif..

*capek deeeee* emg tangan gue scanner? scan di jidat akan keluar nama obat yg alergi? atau punya bola kristal? tinggal tanya bola kristal, pasienku alergi obat apa ya? trus kluar jawabannya? gitu?? so people, please do remember what you are allergic to.. this is very important. if you can't remember the ingredients, just remember the name. or if you can't remember it, write it down in a piece of paper, put it in your wallet or in your note on your mobile phone or your tablet. or even better, keep the wrapping! whatever it is.. please do remember your allergy. blame us if we have already known it and still gave it to you. don't blame us if you don't even know what your allergy is and got allergy reaction after taking the medicine.

the evil matchmaker wannabe
i can't believe i never write about this lady! or wrote about her in my FB status or anywhere else!! hahaha..i must say, this is quite ridiculous story and amusing.
soo.. i was on ICU duty the day i met her. she was admitted to ICU for 24 hours observation after sleep drug intoxication. since i was ordered by the specialist to obtain the reason she drank those sleeping pills, i went to her room when she pressed the "call" button. after asking her need, i asked her about the reason she drank the pills, even though she knew that she had enough already before taking more. she complained about every body parts she could complained and asked to be checked. MRI, CT scan, the expensive ones, you named it, she asked for it. since her specialist wasn't our in house doctor, he couldn't come that morning. so we had to talk to him by phone. and he was surprised by all the complaints and requests. he knew that she was on insurance, so he decided to hurry to the hospital to find out which one is needed so he didn't have to hear all this trash talk again. hahaha..
but during our conversation, she asked me whether i'm married or not. at that time, i forgot whether i'm engaged or just in a relationship, but i wasn't single anymore. so i politely replied that i haven't married yet. she then asked again, if i had someone. and i nodded. her husband told her to stop at that point, but she kept on going, asking if i've been engaged and so on. even she said like this, " as long as she haven't had a ring on her finger, anything could happen under this sky.. (well, in indonesian she didn't say exactly like that, more like "selama janur kuning melengkung, apapun isa terjadi.. yg tunangan aja msh isa putus...) and her husband scolded her harshly upon hearing that comments and apologized to me.
but she didn't stop. she said that if i haven't had anyone, she had someone in mind. her nephew back in US. although he wasn't a doctor, but he has graduated from graduate school. pretty bright future in US, according to her campaign. but i just smiled back at her and then looking for whatever reason i could find to flee from her room.
our first encounter was so funny yet annoying, so i never forget her face. i saw her again back in the ER. i was newly-wed at that time. she accompanied her family member and since i was busy, she just had the chance to ask whether i'm new or not. but few weeks later, she came to ER again. and this time, i helped the ER doctor, so i was around quite enough for giving her the chance to ask me again. this time, she approached me by asking about her papsmear result. but i knew she's just opening the conversation. i guessed she knew that i was about to runaway from the ER so she hurriedly asked whether i'm married or not. here we go again, i thought.
this time i could answer that i'm married. pretty bold i must say, she dared to look at my finger to find my wedding ring. thank God, i wore my wedding ring on the right finger that time (usually i wore it on my second finger, since my ring is bigger than my ring finger..). but that answer didn't stop her. she commented "how shame.. if you haven't been married, i had a nephew in US..you two could make a lovely couple"
aiyaaa.. i almost let go a moan. this nephew of her again! two years and he haven't found someone yet? well then, there must be something with him.. thank God i found mine. hahahaa... udah deh tante.. menyerah deh sonoooo!!!! get lost you and your loser nephew.

what's left for this month
OMG... it's been a month already and i haven't written anything! is my life getting boring or is it me who's become lazier to write? i don't know.. somehow i felt nothing to tell. no interesting stories to write. or perhaps i have already had ears to listen to my stories everyday.. hahaa.. (mekso maksude.. kudu mau diceritai..ehhee.)
let's see.... what's left for this month? all i know.. 2 months in a row, since our wedding, we have received so many wedding invitation. people are rushing to be married, especially this last week. no matter what day it is, don't care whether it's monday or tuesday, or even freaking friday.. people are getting married everyday!! crazy enough.. even on the 11-11-11 KUA had to refuse some couple. hahaa.. i remembered 9-9-09 and 10-10-10 are not as crazy as 11-11-11. fiuhh... luckily we didn't get any invitation on that date. but on the next day, i had 3 invitations.. unfortunately (or fortunately? since i had no idea which one to go?) i had to work.. so no wedding party.. and my best friend's also getting married the next week.. on 19.11.11. hehee.. pretty amazing.. we were best friend since elementary school, and she had crush here and there while i focused on my study. she forced me and introduced me to this guy (whom later become my husband) while we (me and my future husband) ignored her. but then as i started to know my future husband and started to court him, she also started to have a serious relationship with her future husband!
our engagement and wedding were close.. our wedding only a month apart! hahaha.. talking about destiny. weekekke.. let's see if our destiny really amazing.. only one other thing to wait.. hahaha.. and talking about jobs.. hiks.. my job at clinic is getting better. the patient are not as few as couple months before. but one thing for sure.. those working accidents..falling accidents and stuff still keep coming to me. i had to stitch them up with whatever available at the clinic. i always wonder why people keep coming there for first aid even though they knew that hospital were the best option to go for accidents like that. we do have limitation for what we can do at clinic. and they seems ignoring it. blah.. sometimes i wonder, do this clinic really in surabaya? since i felt like there's no different in my story to my friend' at somewhere out there across the sea in other island.

me: *usir* ke RS langsung ajaaa....
pengantar : tolong la bu.. ditolong pertama di sini dulu ajaa...
me: *pasrah* ya udah bawa masuk.
kondisi px delirium.. jelas ada COR karena px tdk sadar, ada muntah, dan skrg aja belon sadar penuh. pas itu px duduk.. keliatan ada fraktur clavicula. hebatnya, para pengantar itu udah aku bilangin untuk ke RS buat CT scan kepala.. tp mereka malah lbh concern ke patah tulang krn itu yg lbh keliatan *capedeee..*

pengantar: ini abis kena mesin circle keramik bu..
me: *not again??* ya udah, masuk dulu, biar saja bersihkan..
alhasil.. jahit menjahit dehhhh... parahnya lagi, pas kontrol 10 hr kmdn, itu orang datang dg kondisi tanpa perban. jadi jahitannya keliatan telanjang mata gituuu.. ya ampun, pdhl kan kerjaane tukang bangunan, banyak debu dan kotoran.. *capede* bagoss.. jahitane jg ga nyambung pula. sipppp...

me: yaa.. kalo dijahit sih bisa pak, tp ini nanti butuh suntik ATS. lha di sini ga ada, jadi nanti saya bawain pengantar, sampeyan kudu nganter ke RS lagi..
*krik..krik..krik..krik* nah lo.. makanyaaa,, laen kali lgsg aja bawa RS kenapa?
tp ada mandor yg bilang gini "soalnya saya tanya teman2, dikasih info di sini ada klinik." lha trus? moso ya rujukan terbaik ke klinik? kudune ke RS langsung ya toooo... plis deh..
ya gak papa lah.. itung2 supaya tmb laris klinik gerejae.. heehe.. duit jg melimpah.. wahahhaa.. *maruk* (balada tongpes abis tur) semogaaa bulan ini jg laris manis... mengingat end of the year is getting near...

untold stories
twitter really makes me lazy to write. i can't believe i haven't written about the last baksos i had. social charity like that almost always crush my heart as i know there are so many people in need but they don't have the money to get the medical care they needed. it's so sad.. and i stumble upon this old man. he was coming alone, with his eyes already deteriorating. but he still could walk slowly and steady. he complained about the itch on his back which he's had it for almost years. when i pulled up his shirt and looked at his back, i was confused. his back is covered with crust and i don't know what the hell it was. it seems like it had been going on several stages. forming vesicles and then burst and dried up, and it was forming vesicles again. and it happened again and again. as if like he's having a pile of roof on his back. pretty scary, and i have no idea what he's having. and neither valen nor andre, whom sitting next to me at the baksos. the only suggestion was giving him something to ease the itch and have him back to the puskesmas and get the referral to RSUDS.
if only i had the information to know what he's having.. and if only i knew the cure. the worst part is, when i suggested him to go to the puskesmas, he told me that he had no one to accompany him anywhere. i have no idea how he could be there. *sigh* this is the irony of this country. people could live in splurge while other live in suffering. hmmpphhh..

after a year and a half
hmm..ndak kerasa aku udah setahun setengah di mitra ini. many experiences, many ups and downs during those times. many cases, many incidences, but i survived though. some may come and go, but i found new friends here. not just among peer doctors, but also from nurses, paramedics and administration clerks. i do like the work environment. although sometimes the schedule is insanely hectic, but since we worked with people that we're comfortable with, it's enjoyable. many funny stories too... let's review some..
comments and questions i heard most in the hospital (since i'm the all-rotating doctor --> ward, icu, emergency room, clinics, even both hotel clinics.. hahaa..talking about being greedy... ):
1. hmm.. u looked new.. i never saw u before: errr... i've been here a year...
2. u look young, how old are you? : errrr... i'm 26 now.
3. u do look young, are u an intern here? nope.. i'm officially a doctor, legal and under oath since october 28th, 2010.
4. are u really have graduated? errr... yeah.. i won't be able to be here and be your doctor if i don't have a license
5. what school? i've graduated from unair. (at this point, some showed awe .. now they're starting to believe that i'm a REAL doctor.. hahahaa..)
6. are u married? nope.. not yet
7. any boyfriend? yes.. we've been engaged (don't think that some will stop at this point, knowing that i've been engaged. a few keep going by saying anything could happen before i'm married.. hahaa.. pretty bold)
8. u're pretty.. don't look like a doctor.. : well, i'll be damn if i have to be ugly to become a doctor. don't judge a book by its cover people...

5. which trainee are u? i'm not a trainee.. i'm a doctor (please.. u've already saw my white coat.. what do u think it was? a cook's coat?)

7. i haven't seen u before... (again... since i'm just filling the blank schedule, it's probably they don't always meet me when they come to the clinic)
and lately, i've been asked for my card.. hahaa.. i don't have one. probably i'll make one soon.

new job..new story? not really
started new job this monday. the coolest part about this new job is, i'm the only doctor there, so i don't have to wear a white coat! haaha.. and since i'm the only doctor there, no more people questioning whether or not i'm the doctor. wkwkwkw.. kinda amazing.. suddenly i'm in charge! hahaha.. bahayanya adalah.. if i wanted to prescribe anything not available there, i don't have my own prescription pad, so i have to use the clinic's instead. and today, another funny story happened. a grandma and her grandchild came to clinic, accompanied by their daughter/mother. they were so funny.. at the end of our conversation, the mother/daughter, asked me about my previous study and stuff. and then she asked for my business card.
mrs : can i have your card please? perhaps if we needed anything, we can call you
me : uummm... sorry..but i don't have a card
mrs: oh.. did you forget to bring it?
me: no, i mean, i don't have a business card.
(gyaaa.. suddenly felt so amateur.. wakakkaa.. )
and talking about shangrila clinic.. things were okay for past two weeks, but this few days there were pretty funny stories.
#1 sad enough, since the suicide bombing at the mosque, the security @ shangrila became more cautious. usually, there were no female security officer at the employer gate, but since the suicide bombing, they put a female officer there to do the body check for female employer. that sucks. now i have to do the body check. the funny part about the female officer, the first one, after checking my bag, where i put my white coat in, asked me where i was going. and then she stopped and realized, "oh..you're the doctor.. please proceed" and she laughed and embarrassed for asking stupid question.
#2 another female officer. the male officer recognized me and he greeted me, " good afternoon.. noon shift, doc?" he knew that for past two weeks, i was on the morning shift. and i replied, "good afternoon, yes sir."
the female officer could've heard us, but then after checking my bag, again.. i put my white coat in it, asked me " are you here for collecting money?"
me: no.. i'm the doctor..
again, she embarrassed and proceed me without asking further. hahhaaa..
when i told the nurses about those two incidents, they laughed and one of them told me that i was lucky since she didn't hand me the "visitor" card. wakakkaa..;
#3 since i've enough body check, i decided to use the front door instead. plus, the HRD office moved near the elevator from the lobby, so today, i decided to use the lobby lift. when i waited the elevator, a man came and asked me, " after an interview?"
me: no..
man: so.. why did u here?
me: i'm going to the clinic.
man: (at the elevator) the clinic in the basement
me: yes, i knew that
i knew that after we reached the basement, there will be another scene, since clinic is turning left from elevator, but i had to go to the HRD first, which was turning right from the elevator. and i was right/ after i stepped out from the elevator and making right turn, he shouted "clinic is this way!"
and i replied "going to HRD first!"
hahaha.... i don't know why things like this happen to me, but i guess it's a refreshment to my boring and hard days.

too young
it happens just a day after my 26th birthday. we got an oncall patient from Shangrila. it's a guy compalining about rash and itch that he's been suffering since yesterday. so when i got there, he was on his way back to his room. since i was wearing my white coat, he knew that i'm the doctor. while he was opening his door, he asked me again, " are you the doctor?" and when i nodded, he replied "too young.." i said, " looks young"
OMG...i'm just getting older the day before and he said that i'm too young? oh damn.. probably those anti-aging factories should find me, and probably i could be their best model. looking young without anything!
but couple days ago, when i waited for the psycho test interview, we exchanged stories with other candidates. i was so relieved when i found out that it's not only me that get underestimated. kakak kelasku itu cewe berjilbab yg tampangnya cukup keibuan menurutku, dan dia bilang bahkan dia sendiri masih diragukan kalau dia itu dokternya. wow..that's a relief! padahal aku sudah sering berusaha tampak dewasa pas menghadapi pasien2 itu, tp mukaku ttp kelihatan kayak anak SMA. *sigh* some says it's an advantage, but in my case, it's a disadvantage. but somehow, knowing that others also struggle with their appearance, it brings me relief.

appreciate life people!
sehat itu mahal. tp kadang org jg tdk bersyukur saat msh dikasih kesempatan utk memperbaiki kesehatannya dg diberi penyakit. i know, getting hospitalized nowadays cost a fortune, especially if you wanted to get the best care from the best specialist. biarpun ada asuransi, tdk menjamin semua akan ditanggung pihak asuransi. let me use an example.
several months ago, there was this patient, an 80 yrs old woman, looked humble in ICCU. which is a rare view in our ICCU. out of curiosity, we asked her, since according to her record, she'd had a stent inserted in her coronary artery which cost her more than 70 million rupiah. we're not underestimated her, but we just curios since most patients, especially the uneducated ones, refuse to get a stent when they found out the cost. she told us that she only had 2 kids, one son and one daughter. her son opened a garage near their home. while her daughter was a worker in some factories. since the regulations in most factories never cover any family member if the worker is female, we wondered where she got all the money to pay for the stent. but i guess, her children could manage to get it so she could get the best care.
while couple weeks ago, a 56 year old male, a cook from renowned hotel in Surabaya being hospitalized because of heart attack. we caught it early and we offered to do angiography and probably a stent since it was the best therapy for him. the management from the hotel agreed to do it as long as it is necessary to safe his life. but the added cost for his care will be discussed later after he's being dismissed. when he heard that he had to pay any extra cost for his care, for more than 10 millions, he refused the angiography and prefer the streptase which is cheaper.
from this two cases, i found out a different decision. probably both families didn't have much money. but the first one, manage to find a way to collect that much money so their mother could be treated to the best and get the best result. while the second family, even though he's still working and probably he could save and pay the management a little by a little, but he refused it. i don't know if he had other debt or any other consideration that made him refuse the angiography that time, but compared to other family, i wonder, why he couldn't manage it? padahal lek dilihat, dr luar tampak lebih mampu keluarga kedua drpd yg pertama.
and he seemed like ignoring God's warning. stl serangan jantung, apalg dg hasil yg seperti itu, harusnya dia ga blh kerja berat dulu. but only after a week, he got back to work. i don't know. kadang orang2 seakan nggak mengerti kalau mereka itu msh disayang Tuhan. masih diberi kesempatan utk memperbaiki hidup. masih diberi nyawa sambungan. dan makin sebel kalau ternyata org tersebut malah memperburuk keadaannya dg tidak menaati aturan rumah sakit dan malah seenaknya sendiri.

RIP: Mrs.S
RIP. i might be cruel for predicting her fate before it became real. but watching her record, she will eventually dead sooner or later due to her stubbornness. and it became reality this morning. she came back to ER too late. she had been died in her house only God knows when. her family rushed her in to our ER, but it was too late. they couldn't do anything.
pretty sad though. they had money. they had the chance to get her better. but every time we suggested this and that, they always refuse.they always bargain. i know, as a doctor, probably we should push them to obey our advice. but as human beings, we couldn't force them to obey us. and there's a freedom of choice. they chose not to obey our advice, so as the same human being, we have to honor their choice. too bad though, because we knew that their choice is wrong. we, doctors, we knew better what's best for them. but we should try our best to make them understand it.
but once again, only God can do miracle. even though we have tried to convince them, we might fail. only God that can influence their decision. we could only try our best. perhaps, yesterday i wasn't try hard enough. perhaps i should force her to stay in the hospital. perhaps i should scare them, so they didn't bring her home. but again, they made their choice. they had choose her fate. she had choose her fate. may her rest in peace.

dilemma
sekarang aku jd menyadari, jd dokter di masa ini susah. we're facing a new problem. educating people about health prevention and helping them to be aware of their symptoms. but then it backfired at us. kadang ada beberapa org yg psikosomatis, malah memanfaatkan ilmu yg kita berikan ke mereka sebagai keluhan dan they can mimic the symptoms so we doubt whether it's the true symptoms or not. bbrp malah terlalu panik sehingga tiap keluhan muncul dijadikan masalah besar dan minta perhatian sesegera mungkin. even though we've explained that it's not emergency, they couldn't understand it. more over, they are people with money, so they think their money should get our attention to give them priority.
but talking about money, i realize that people with money, they all very demanding. some demand priority, assuming that money should give them priority. some obey our rule and instructions, but others disobey us, thinking that they're smarter than us and they can instruct us which medication they would follow and which is not. this is ridiculous.. kita para dokter terancam hukuman jika kita melakukan malpraktek. what if, malpraktek itu terjd krn mereka yg keras kepala? tidak mau menuruti segala instruksi kita dan menganggap kita ini berlebihan? dan nanti pada saat yg kita pikirkan itu benar, mereka balik menuntut kita dg kelalaian?
meet mrs. Sul and her family. she suffered chronic kidney disease. since the first time she's been diagnosed, the doctor has instructed her to do hemodialysis.but she refused. she bargained. she just wanted the diuretic injection. from time to time, every time she had difficulty in breathing, she would go to ER and quarreled again with us, refused to be admitted to the hospital and then ended up going home after being injected with diuretics. and one day, the diuretics couldn't help anymore. by the time she agreed to do the hemodialysis, it's already too late. the access to her iv were a mess, so we couldn't do it. we had to consult to a vascular surgery to insert a trilumen. Then before she had the dialysis, she couldn't breathe so we intubated her. after few days in the ICU and then regular hemodialysis, she's still stubborn.
AV shunt didn't work out. her trilumen is not working either. so she got no other choice. she had to get hemodialysis, but no access to do it. the other option is CAPD, but then, it couldn't be done soon. she refused to be admitted, but her physics couldn't hold longer. the family didn't help either. they blamed us for not cooperating with all their demands. as institution, we're held back by regulations and protocols, tapi mereka seakan memutar balikkan semua peraturan sehingga terlihat seakan kita yg tidak berperikemanusiaan. mereka yg menolak segala terapi, mengatur semua tindakan, menolak semua anjuran dokter. if i wanted to be rude, aku bakal bilang ke mereka "kalo gitu ngapain ke rumah sakit? kalo minta ditolong, harusnya nurut semua. kalo masih ngatur, ya obatin aja sendiri"
and now his husband cursed me for being cold-blooded doctor. he's the one that still hardened his heart and chose to comply his wife and brought her home despite her breathlessness. aarrrgghhh...what a stubborn, rock-headed family. abis gitu masih nyalahin semua orang. perhaps it's the curse from the nurses back in ICU. krn mereka selalu menyalahkan kita dan terus tidak mau kooperatif dalam tiap tahap terapi, akhirnya para perawat yakin, she wouldn't survive. bukannya kita mendoakan dia meninggal, but according to our record, people like her and her family, ended up dead because of their stubbornness. let's see whether or not she will survive. may God give revelation to her and her family.

ER dilemma
ini akibat peran dobel, jadi anak pendeta sekaligus dokter jaga. nasib..nasib... dulu awale bingung gimana hrs bersikap lek ngadepi pasien jemaat. apalg kalo mereka2 jemaat lama seng kenal aku dr kecil. bingung gimana ngadepinnya. lha wong mrk udah kenal aku dr jaman masih imut2 dulu, dr masih mbrangkang, ampe skrg udah seperempat abad. rasae sungkan ae kalo aku kudu mengedukasi mereka. mereka lo tau mokong2e aku gimana pas kecil..trus skrg jd dokter yg meriksa mereka... hmmm..lek aku jd mereka aku dewe jg bakal merasa.."ini kan anak ko den yg dulu msh selutut..eh..skrg udah jd dokter.. " rada separo ga isa nyalahin lek mereka ga mau takperiksa atau ga percaya..
tp lama2 aku bonek aja. sok pede kalo ada jemaat. lama2 mrk jg udah biasa. biarpun kadang msh ngeledek "bu dokter" tp aku cuek2in aja wes. soalnya resiko kalo diliat pasien lain, ntar mereka kira aku ga pede. masalah yg itu beres, tp muncul masalah lain. soal fee. di satu sisi, sebagai makhluk Tuhan yg masih normal, siapa sih yg ga butuh duit? even a pastor's daughter also needs money. wekekekeke... tp di sisi lain, masa anak pendeta ga beramal? narik duit dari jemaatnya sendiri?? errrr... aku jd bingung. akire aku membuat keputusan sendiri. SK ala eunike. ehehhehe... kalo mereka baru pertama kali takperiksa dan emg kenal deket, takgratisin, apalg kalo ga mampu. tp kalo yg tajir2 trus ga kenal2 amat apalg kalo recordnya nyebelin, ya bayar aja deh.. wekekkeke.. anggap aja persembahan, hahahahaa...
tp masalahnya adalah, tiap aku menihilkan honor dokter, bagian counter tuh mesti nanya. kalo alasane krn jemaat terus, jangan2 ntar malah ga boleh dinihilin. guawat dongg..secara jemaatku kan banyak tuh. repot nih. apalg pas kmrn jaga malem. entah knp seakan jadi hari jemaat sedunia. dateng2 udah ada anake tiky yg mo nebul. abis gitu tengah malem ada cucue bu umar 2 orang. haiss.. akire yg cucue bu umar takksh buy 1 get 1 free.. wakkakaa.. soale pasienku dikit..lek digratisin semua, aku gigit jari. lek ga takgratisin, wong bu umar itu baik ama mami...serba salah. akire buy 1 get 1 free ae wes. pagie juga gitu, ada mas2 nyerempet ibu2. krn perawate juga sakno, akire rawat luka ama honor dokter pun dicharge buy 1 get 1 free. obate jg takksh yg murah2.
nah, urusan fee udah ada SKnya. skrg urusan lain. sometimes, even though they are my congregation member, sometimes they could be pain in the ass. mulai dr yg minta diutamain, ampe yg ga nurut lek disuruh ngurus admin. sebel deh.. perawate jd keki. aku jg sungkan. krn di satu sisi aku kenal mereka, tp krn mereka ga kooperatif, aku dewe jg ga enak ama perawate. grrrr... kalo udh begini ini..bayar aja deh. biarpun kasihan, tp kalo annoying ya males bgt ngeladeni. aku dewe jg kalo mereka komplain, selama masih masuk akal ya aku ladeni. tp lek wes ketok mrk yg kenemenen, mek masuk kuping kiri keluar kuping kanan. masa bodo amat aku dirasani di belakang. kadang aku jd risih sendiri. mereka lo jemaatku, tp ternyt kalo di luar they act bossy and stuff, pdhl kalo di gereja they act like they need help, especially in economic. grrrr... jd pengen marah kalo liat begituan. *sigh* ya udah lah.. namanya jg manusia... diterima saja....

SK ala eunike. ehehhehe... kalo mereka baru pertama kali takperiksa dan emg kenal deket, takgratisin, apalg kalo ga mampu. tp kalo yg tajir2 trus ga kenal2 amat apalg kalo recordnya nyebelin, ya bayar aja deh.. wekekkeke.. anggap aja persembahan, hahahahaa...

too deep
emergencies kept coming into my ER door last monday night. i was exhausted and so the nurses. but we couldn't refuse all the patients. we kept helping them. pacing with time, and we worked as fast as we could so we did no harm to our patients. in our mind, we also hoped that if we could finish earlier, maybe we could steal a minute or two to rest before another patients came in. but that's just our hope. people kept coming without any rest. the last emergency patient was a 75 years old male. his family found him lying in the bedroom floor and they noticed that there's a bump in his right forehead and they couldn't wake him up. so they rushed to my ER, because he had previous history of intracranial bleeding.
unfortunately, as we did the CT, we found another bleeding (again) and this time, it was more than before. and operation was no longer an option. it was the only option to save him. i tried as fast as i could to give that information to the family without worried them any longer. i tried to get their approval without hurting their feelings. but perhaps God had another plan. the OK team came late and the neurosurgeon had left because the OK wasn't ready. the anesthesiologist also came late. in short, my patient didn't survive.
i was angry and disappointed. it was a long and hard night. i tried as hard as i could to save him, to make him proper for operation. but he didn't survive at last. i know the prognosis for him wasn't good since it was his second attack and he was old. but i thought there will be a small chance that he could survive if we operated on him sooner. that day, i cried everytime i remembered him. and i realised, i'm going in too deep. i'm not supposed to let my emotion clouded my judgement. what can i say? i was exhausted and the bad news at the end of my duty really ruin my day. perhaps tomorrow will be a better day. people have flaws and nobody is perfect. just do my best and keep improving.

it ain't easy
breaking bad news to patient and his family is not easy. breaking bad news to fellow colleague and his family is even harder.
dr.M rushed through my ER door along with her physiotherapist. in front of them is an old woman, holding a baby girl. she was blue and pale. i thought she had convulsion. but i was wrong. she wasn't breathing and her heart stop beating. we hurried, giving her high flow oxygen and chest thrust while the other searching for her vein to insert the IV line. we set the monitor, but nothing. it's still a flat line. we keep trying. a few attempts failed to put an IV. i called a pediatrician in his clinic to help me in the ER. he also rushed in and after he arrived, he decided to intubate without waiting the anesthesiologist. after he intubated, nothing changed. her oxygen saturation keep decreasing. she's still bluish and pale. adrenaline was given. twice. third time. nothing happen. IV line inserted, but it didn't change anything. she didn't give any response. after 30 minutes trying to save her, the pediatrician stopped our resuscitation and declared her dead.
the harder part is her uncle is my college-mate. he was 2 years older than me. i knew him. it was difficult to explain about the administration and stuff. while he had worse job to do. he had to tell her sister that her daughter has died. and she died in front of her own eyes. in her arms. she stopped breathing in her hug. and it's hard not to shed tears in front of them.
it ain't easy. even though i've done it many times before. telling patients or their family. telling their family that their loved one is in critical condition and probably won't make it. and then declaring them dead isn't easy. and it's not getting easier.
we doctors, we also human being. but we're demanded not to show it. but deep down inside, it crushed our heart. every single life, every heart beat. if it's possible, we will try as hard as we can to keep it.

Name, face, and status
Working in the ER besides pumping adrenaline, it's also refreshing sometimes. Watching all kinds of people coming to ER and all their personality, it's really interesting dealing with it. Like tonight, there was this man who came to the ER because his wife cut her wrist while she was washing the dishes. As he waited me to stitch her up, he saw my name written on the "on duty" board. He predicted that i'm a batak.. According to my name which is taken from the Bible. And my middle name is also interesting for him. While my last name didn't show on board because my name is long enough so there wasn't any more space, so he just guessed that my last name is my family name. A Batak family name, hahahaha... When he said that to me, i already explained to him that he was wrong. But he probably didn't believe me until he heard me talk to my fellow in javanese. Then he said that after hearing me speaking javanese, he believed that i'm a local, not a Batak. Aahahhaaa... Pretty stubborn man...
Problems also arise from my racial status. My face is clearly showing that i'm half chinese. But since i was born, no one talk in chinese so i can't talk chinese. I do took some lesson but i stopped half way, so i could understand some simple phrases but i couldn't talk back to them. A man coming to ER to translate a chinese family and he pointed to me that as a half chinese i should learn to speak the language. I just grinned back at him, because i do have the intention to learn chinese again, it's just the time that i don't have. This face has caused me some trouble because when they saw me, they thought i could speak chinese and they just talked in full speed while i couldn't understand what they're saying if they spoke that fast. *sigh* thank God i have my iphone to translate and help me communicating with the non English-speaking patients.
And my status as a pastor's daughter also cause some problems. If i didn't free the charge, they could badmouthing me and my family behind our back. Saying that we're greedy, not tolerant to the congregation's member. But if i gave them all free of charge, the hospital administration will ask my reason. I couldn't say that they were all my congregation's member as an excuse everytime coz it may be the hospital loss. But what can i do? For now, i have my own policy. Good friend of mine, without insurance, good people back at church, moderate income or lower, on their first visit, i'll free them off charge. But other than that, they have to pay. Fair enough?

sure life is an unfair world
one of our emergency nurse is being admitted due to pneumonia. she had fever for more than a week, cough crazily for about a week and she already had a blood count done, and the result came back normal. but she still had fever up until today. she got the night duty yesterday, and at the end of her shift, the doctor in charge suggested to her to get an X-ray, and the result is pretty shocking for me. she got pneumonia.
she's 25 years old, same age as me, still single and underweight (based on her appearance). she worked her ass off in the ER and she worked really hard. multiple overtime, crazy ambulance tasks, and God knows what else she has been dealing with in our ER. and now, she's sick. pneumonia...oh gosh..that's something you can catch from patients that came to ER. but that's scarier than just upper respiratory tract infection.
and it got me thinking. as a medical provider, we're at risk of catching something from our patients. unfortunately, here, we don't get the protection that we need. there are a lot of rules and conditions before our company paid our expenses. and hell with the universal precautions. here in indonesia, even though all the international-labelled hospital already tried to do the best they can to practice the universal precaution as standard procedure, unfortunately, there were holes and cheats here and there. no one here in indonesia that do the universal precaution 100 % as standard procedure. it's different from abroad where everyone is so strict about it. they are so afraid of spreading infections and stuff. and don't forget the patients.
patients here clueless about what they could do to us. they forgot that we're also human, that we can be sick too. that we can be infected from anything that they had the moment they came into our ER. the moment we treated them. the moment they withhold important informations from us. patients abroad very meticulous about their complaints. they listed their complaints and problems. any previous history they told us all. they understood that anything could affect our judgements if they withheld some informations.
but here? we, as doctors and nurses, are required to be semi mind-reader and fortune teller. we need to have the ability to scan the diagnosis out of the patients just by watching them from head to toe as they might not reveal everything. and we have to play some tricks in order to get all the right informations from them. and the most dangerous part is, here, we have more HIV cases every year. it's getting nearer to our home. but since it's taboo to reveal that you have HIV, some patients just withhold that important information from us just because they're embarrassed about it. which is sucks.
me, myself, am a sloppy person. i got bruises and scratches and cuts here and there. i bumped into things more than anyone else. i got cut from nearly everything. i'm at chance of having micro cuts that i might forget, that i might not know i'm having it, that i might not feel when i got it. which means i'm at risk of fluid exchange if i didn't wear gloves or goggles or anything else written in the universal precaution' procedure. which means, damn...i might get infected if i still being reckless. *sigh*
it's hard to become a health provider. we offered our help to help others. we're at risk everyday. we seldom get the gratitude from those that we've helped. we often get complaints from simple mistakes that any human could do. and we get minimum protection from our company. sure life is an unfair world.
learn from mistakes
#1 miss A: she came to ER 5 minutes before my shift ended. she complained of abdominal discomfort, she felt nauseous and kept vomiting even though she's already had treatment the day before. since i wanted to go home soon, i checked her real quick and suggested that she should be admitted. since she showed no specific signs of any specific conditions, i admitted her as dyspepsia case and being treated by an internist. the internist came in the afternoon and examined her, also found nothing specific and ordered USG for the next day. turned out she had perforated appendicitis! gosh... and then, the problem didn't stop there. after she's being discharged, she came back again because there's pus from her operation site. and her case became worse. she needed the second surgery and stuff... hufff... really... pretty complicated from an unspecific case.
#2 mr.S: he also came at injury time. i already exhausted. and the moment he came to ER, i already knew that this won't be easy. multiple trauma case. and i needed to act quick. so i consulted to a neurosurgeon that still in the hospital at that moment. unfortunately, since he needed a lot of examination, and he got multiple lacerations plus he's delirious, it was hard to stitch him up. it took me more than an hour to stitch him up. and then, i couldn't pass him to the morning shift because she had another trauma case also. pretty hectic morning. so, in my exhausting state, i consulted him to various doctors. and then, as we evaluated again what has been done and what hasn't been, it turned out that we haven't done the USG. the nurse told me that they could do the USG in the ICU, so after stitched him up, the nurse took him straight to ICU as he still had to wait the radiologist to get the ultrasound. again...turned out he had hepatic rupture... :( and he had to stay lonnggggg in the ICU until the family decided to take him to another hospital which was cheaper.
#3 mr.A :this is probably the worst case so far. he came to ER for getting x-rays after falling down a day before. since there were problems with his company insurance, and he couldn't afford to pay it first, so he decided to go home and won't back to hospital even though i had called him to get back to the hospital because he probably needed more examinations and probably an abnormal results and he needed to be admitted. he refused. and i don't know till today if his result really came back abnormal. so...this is a slap in my face to be more careful. and listen to my gut.

my new world
yep.. now i'm in adults world, which is sucks. got a new job, pretty fast though, remembering what i put in my resume is not quite impressing. i don't know why but i got accepted, which..thank God for that. i was applying to mitra waru at first, but then the medic manager saw my address in the resume, she offered me to get transfered to mitra satelit. at that time, my only concern is getting a job so even though they said that it's though in mitra satelit, i didn't listen to them and just agreed to the idea.
turned out that working in mitra is incredibly insane. people are demanding. pretty exciting at first, but also frustating. there's a morning report, two night duties in a row every week, an on call, referring patients, and hotel clinic. lots of job huh? but it's interesting. the not interesting part is the payment. pretty minimum for a doctor, but as a newby, i don't expect much anyway. but i'm hoping that my salary will increase as time goes by.
and then there's a degree here. ward doctors, icu doctors and emergency room doctors. the ward is the lowest degree and the emergency is the highest. you have to pass the ward first and then graduated to icu before finally being in the emergency room. being on duty at emergency is fascinating but also scary. i'd like to be in the emergency room, but sometime i don't like to be alone there. having a partner to back you up, that's what i need sometimes. somehow, i'm still not confident enough facing all the patients coming at the emergency ward. but until now, i'm still trying to be one.
so..here i am, in my new world. just trying to fit in and hopefully i will be the REAL doctor soon. (not that i'm not..but sometimes i feel that i'm not one)
